FCGB Registration Forms

When a player has received an offer to come play for a FC Green Bay State or Classic League team and the offer has been accepted, all forms listed below have to be printed, completed and turned in. Please register on-line first and select your form of payment (credit/debit card in full or debit/credit card payment plan). Bring all forms listed below along to the team meeting. Team meeting dates will be listed on Team Meetings page, once a date is set by the team rep/manager.

If you are absolutely unable to attend the team meeting, please make arrangements with the assigned team representative to have the forms delivered, before the team meeting date.
Remember, your player registration is not fully completed until you have:
1. Completed the Online Registration and
2. Turned in the completed forms below.


If you are a player that has been added to a roster after the team meeting date, please send all forms to the address listed below unless otherwise instructed by your team representative. You must also register online. Please write your specific team age, sex and team color after "Player Registration" on the envelope, and please don't forget to send a picture with the player's Name and Team (Gender, Age and Color) to:

FCGB 2010-2011 Player Registration - Additional Forms

Please select,  print and fill in the four additional forms below (marked with an asterisk*) and bring them to the team meeting.  The Aurora Notice of Privacy Practices is for your information and does not require your signature. See Tryout Schedule for team meeting dates and times.

New Players to FCGB should also bring a copy of their Birth Certificate or Passport to the team meeting.


It is the goal of FC Green Bay that financial hardship should not prevent talented and committed players from participating in the program. This scholarship was established to provide assistance to those young people with a demonstrated financial need and a commitment to soccer.
