Managers Checklist

Early Registration: Fondy Soccer Complex (402 Campus Drive) Friday afternoon August 25th 3:00-5:00 PM or at the Radisson Hotel & Conference Center (625 W Rolling Meadows Dr) - Fond du Lac Boardroom #1 on Friday evening 6:30-8:30 PM
Saturday Registration: Registration Tent, Fond du Lac Soccer Complex 1 hour before your first game - Saturday Registration will start at 7:00 AM for matches beginning at 8:00 AM

Please bring the following items with you:

Copies of Official/Approved Team Roster - (please have Uniform #s that match players on your roster)
Copy of Roster for Club Pass Players (only if using Club Pass players within your club)
WI Release of Liability and Consent Form - (needed for each Player, including Guest Players)
Your State Guest Player Form, only if applicable
Your State Permission to Travel form required for out-of-state teams only

Tournament Fee:

Tournament fee can be paid on Saturday at Registration prior to your first match, or make check payable & send to:
